Initial Telehealth Consultation

$150 for a 45-minute session, which includes a comprehensive health assessment, consultation, personalized treatment plan and access to patient portal via website or IOS app.

Follow-Up Telehealth Appointments

$60 for a 30-minute follow-up consultation to assess progress and adjust treatment plans as needed.

Initial In-Home Consultation & Treatment

$350 for a 60-minute visit, which includes a detailed health assessment, physical exam, personalized treatment plan and access to patient portal via website or IOS app.

$160 for a 30 minute follow up to complete physical examination and personalized treatment plan.

Interventional Pain Management

Procedures such as nerve blocks, epidural injections, and other minimally invasive techniques are referred to pain management for best treatment options.

Expertise in prescribing and monitoring pain relief medications tailored to your specific situation.

Physical Therapy

Specialized programs designed to enhance recovery and improve quality of life, including referrals to therapy and the provision of home exercise programs, or a combination of both.
