Our telehealth and home visits are direct self-pay, allowing you to receive personalized care without the hassel from insurances. However, all treatments such as medications, and referrals, are billable through your insurance provider when applicable.

Initial Telehealth Consultation
$150 for a 45-minute session, which includes consultation, comprehensive health assessment, personalized treatment plan, patient portal access, IOS and Android Elation app access.
Follow-Up Telehealth Appointments
$60 for a 30-minute follow-up consultation to assess progress and adjust treatment plans as needed.
Initial In-Home Consultation & Treatment
$350 for a 60-minute visit, which includes a comprehensive health assessment, physical exam, personalized treatment plan, patient portal access, IOS and Android Elation app access.
Follow-Up In-Home - Physical Exam, Treatment & Referrals
$160 for a 30 minute follow up – post initial telehealth consultation to complete physical examination and personalized treatment plan.
